1.
The licensee shall obtain all infomation from the foreign exporter necessary 1-to complete the shipper's portion of Form NRC-741, " Nuclear Material Trans-1 actions Report."
If the licensee is the receiver of the material, he shall

record both the shipper's and receiver's information on the Form NRC-741 and f,
transmit pursuant to Section 40.64 of 10 CFR Part 40.
If the licensee is not s*
the receiver of the material, he shall provide the shipper's information to the receiver of the material for inclusion on the receiver's Form NRC-741.

The licensee shall notify the appropriate regional Safeguards Branch, t
Office of Inspection and Enforcement, imediately, by telephone or telegram, 3
i if entry of any import shipment authorized by this license is refused by the
