O Colt Industries Trent Tube Division of Crucible, Inc.

Docket No. 90900314/81-01 NOTICE OC NONCONFORMANCE Based on the results of an NRC ir.spection conducted on February 9-12, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstames and shall be accomplished in accord-ance with these instructions, prucedure., or drawings.

Instructions, procedures, or drawings shal! include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

The following nonconformances were identified:

A.

The QA Manual, paragraph 17.6 states in part, " Calibration shall be per-formed by the personnel or a subcontractor.

The Calibrator shall upon completion of satisfactory calibration place a calibration sticker (Exhibit 17C) on the device, entering on the sticker the dates when the device was last calibrated and when the calibration is due, and initialing the sticker.

For small instruments the sticker may be placed on the box...."

l Contrary to thi above:

A completed sticker was not attached to either calibrated optical pyrometer, SN R-22428, or its box.

Prior to the end of the inspec-tion, corrective action was provided by attaching a completed sticker to the pyrometer instrument box.

2.

The calibration sticker on a hardness tester, SN 87481, did not con-tain the correct date of last calibration or the due date, in that:

a.

The Certificate of Calibration for the instrument showed that calibration had been last pe.cformed by a subcontractor on l

June 2, 1980, with a due date of June 1, 1981.

b.

The calibration sticker on the instrument showed a Trent Tube calibration date of November 25, 1980, with a due date of February 25, 1981.

B.

The QA Manual, paragraph 12.2 states in part, "The Mill Order (Exhibit 7A) is the master document for controlling operations... and processes in the shop.

It performs the following functions:

.. Identifies procedures with revision numbers to be rollowed for welding...."

2 Contrary to the above, the welding procedure specification revision numbers were not shown on two in process mill orders (Nos. M.0. 5-C5-35067-1-0 and M.0. 3-C5-50469-0-0) and on two completed mill orders (M.0. 2-C5-50514-0-0 and M.G. 3-C5-50469-0-0).

C.

The QA Manual, paragraph 13.6.1 states in part, "A welding technician assigned to the welding work station is responsible for reviewing the Welding Report and checking the following:

(a) The correct revision of the welding procedure is available at the work station;..."

Contrary to the above, during welding of ASME Code, SA-312, T-304L piping for order M.0. 5-C5-35067-1-0 and ASTM A-312, T-304 stock piping for M.0. 4-C3-01.563-0-0, obsolete revisions of the welding procedures were at the work stations (WPS TWP-22A, Revision 1 instead of Revision 2 for M.0. 5-C5-35067-1-0, and WPS TWP-8A, Revision 1 instead of Revision 3 for M.0. 4-C3-01563-0-0).
