Underrated Terminal Strips on Limitorque, Valve Operators INTERIM REPORT 2 DATE:
April 10, 1981 PROJECT:
Consumers Power Company Midland Plant Units 1 and 2 Bechtel Job 7220 Status of Corrective Action and Investigation The status of actions in progress for resolving the subject MCAR is as follows.
1.
Henry Pratt Co. has been requested to provide Sechtel with the following information with supporting documents from Limitorque Co.,
justifying the use of Marathon 300 Series terminal block.
a.
Actual test records for the Marathon 300 Series terminal block which established that this block is suitable for 460 V opera-tion b.
Identification and rating of the terminal blocks (manufacturer, model number) Limitorque is currently using for terminating the leads for the 460 V motor e
c.
Basis for upgrading the Marathon 100 Series terminal block from 150 to 250 V rating.
d.
Identification of all types of terminal blocks (manufacturer's model number, voltage rating and specific method of identifying one type from another) which Limitorque may have used and also the acceptance criteria used to determine their suitability for the intended function 2.
Seven vendors (covering 11 different orders of the Q-listed valves with Limitorque operators) have been informed that several Limitor-que operators with underrated terminal blocks, which were supplied under one of our purchase orders, have been located in safety-related systems. This condition has been reported by Consumers Power Company to the NRC under 10 CFR 50.55(e).
All vendors have been apprised of their legal and contractual reporting obligations under 10 CFR 21.

All seven vendors including Henry Pratt Co., have been requested to arrange for an inspection of their operators. One-hundred percent of the valves from Henry Pratt Co. on Purchase Order 7220-M-132(Q) are to be inspected. On the rest of the Q-listed orders, the in-spection will be conducted by e. random sampling program. If an underrated terminal block is located, an inspection of 100% of all orders (Q-listed and non-Q-listed) will be performed.

Bechtel supplier quality representatives have been instructed in and provided with the guidelines for the terminal block in-spection of any unshipped Limitorque operators to be inspected at the vendor's shop.
Our next report on this subject will be issued by June 19, 1981. At that time, results of the completed inspections should be available and a schedule for those remaining could be established.
