Description of Event

On March 28, 1981, with the unit in Mode 5, the primary grade water isolation valve to the blender,1-CH-217, remained open approximately one hour after securing makeup flow to the RWST. This is contrary to T.S. 3.1.1.3.2 and reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occurrence Since isolation was provided between the primary grade water supply and the blender by valve FCV-114A the health and safety of the general public were not affected.

Cause of Event

The valve had been inadvertantly left open after securing makeup flow to the RWST.

Immediate Corrective Action

rm When it was realized that 1-CH-217 had been lef t open for approx-(_)

imately one hour af ter securing flow to the '.!WST, it was immediately closed and locked.

Scheduled Corrective Action l

The procedure that controls makeup to the RWST from. the blender contains instructions to close 1-CH-217 within 15 minutes with double verification at the end of the procedure. This evolution demonstrated th2t the makeup process will be intermittent; therefore, a caution will l

be added to the instruction that opens the valve to insure its closure l

within 15 minutes af ter makeup is ter.inated.
