Attachment ITEM A COMMENTS:

Please add the following paragraph to the response provided under the Notice _of Violation, Item A: ACTION TAKEN AND RESULTS ACHIEVED.

S&W FQC has instructed all concrete inspectors to assure lighting in concrete placements is adequate to assure proper placement and inspection, and to immediately contact their supervisors if problems with securing addit: 2nal lighting are encountered.

ITEM B COMMENTS:

The aggregate water quantity of 728 lbs. shown on batch ticket no.

14335 is slightly higher than actual quantity of 706 lbs., because it is calculated by the batch plant computer as the moisture content times the quantity batched, rather than moisture content times the dry weight of the quantity batched. Thus by calculating more water than is actually added at the plant, the calculated allowable water is lower than actual. This practice provides for a small safety factor to help prevent exceeding the water cement ratio when the amount of-water added exceeds the apparent amount withheld as in this case.

Action Taken To Prevent Further Violations:

FQC Personnel have been instructed to check all tickets prior to discharge of the load of concrete for maximum water allowed. No load will be discharged until this is done. If maximum water is exceeded, the load will be rejected immediately.
