Grand Gulf Nuclear Station Units 1 and 2 Docket Nos. 50-416/417 File 0260/15525/15526 PRD-81/09, Status Report #1, Unscheduled SIS Cable Pull AECM-81/105 On February 11, 1981, Mississippi Power & Light Company notified Mr. J.

Rausch, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Statimn (GGNS) construction site.

The deficiency concerns the installation of unscheceled Single Insulated Stranded (SIS) wire without a specific Work Plan and Inspection Report (WPLIR) or documentation.

Our investigation as to reportability of this matter under 10CFR50.55(e) and 10CFR21 is continuing.

Our status report is attached.

We expect to file our final report on this matter on or before September 9, 1981.

Attachment to AECM-81/105 Page 1 of 1 STATUS REPORT #1 TO PRD-81/09 I.

Description of the Deficiency Valves were received and installed with unwired auxiliary devices. To connect these devices, terminal boxes were installed in the field in accordance with Constructor specification E-0725 f or the control cable teraination, and flexible conduits were branched off with unscheduled (normal computer routing card was not utilized) and subsequently the wire installation was undocumented and not inspected as presently required by Work Plan / Procedure (WP/P)-E22.

The systems affected were:

B21 E51 G41 P21 PS2 P66 T42 E12 E61 G46 P41 P53 P71 T51 E21 G33 M41 P44 P60 P75 E22 G36 P11 P45 P64 T41 This deficiency is applicable to Units I and common devices shared by Unit II.

Investigation is underway as to reportability under both 10CFR50.55(e) and 10CFR21. The affect, had the deficiency re=ained uncorrected, is under investigation.

Our Architect / Engineer is currently evaluating the deficiency as noted and its safety implications.

II.

Approach to Resolution of the Problem WP/P-E-22 procedures did not provide for documenting the installation and inspection of unscheduled SIS wire at the time the deficiency occurred. WP/P-E-22 was revised prior to the reporting of this deficiency to establish criteria for documenting the installation of unscheduled SIS wire.

WP& irs have been issued to identify the location of approximately 220 valves for inspection of the deficient SIS wire installations.

Status of Proposed Resolution Our Architect / Engineer will complete his evaluation after receipt of the documented results of the resinspection.

IV.

Reason Why a Final Report Will be Delayed The reinspection and documentation have not been completed.

Date When Final Report Will Be Submitted Our final report will be submitted to you on or before September 9, 1981.
