O APPENDIX A NOTICE OF VIOLATION Mercy Hospital Docket No. 30-1804 Springfield, Massachusetts License No. 20-00096-02 Based on the results of an NRC inspection conducted on October 2,1980, it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your license as indicated below:
A.
10 CFR 20.203(4) requires that prior to disposal of an empty uncontam-inated container to unrestricted areas, each licensee remove cr deface the radioactive material labels or otherwise clearly indicate that the container no longer contains radioactive materials.
Contrary to this requirement, on the date of the inspection, October 2, 1980, labels on containers were not defaced prior to disposal to an unrestricted area.
This is an infraction.
B.
Condition 19 of your
-se requires that licensed material be possessed Ond used in accord-
.h statements, representations and procedures contained in your appeication dated August 30, 1977 and various letters, one of which is dated February 27, 1978.
Item 5 of this letter requires the responsibilities, duties and meeting frecuency of your Medical Isotopes Committee be as in Appendix 8 to the NRC's Guide for Preparation of Applications for Medical Programs.
Appendix B requires quarterly meetings of the Medical Isotope Committee.
Contrary to the above recuirement, as of October 2,1980, the Committee did not meet from Novem.'.ar 21, 1979 to July 3, 1980, a period greater than one quarter.
This is an infraction.
