SECOND INTERIM REPORT Description of Deficiency During installation of the gas stripper pump cedule (GSPM) at Yellow Creek Nuclear Plant, a TVA welding inspector noticed several weld deficiencies. For example, fillet welds were found to be undersined

-with some visible porcsity. There also a;; eared te be a lack of penetration on butt welds. Arc strikes were visible. The GSPM is car.ufactured by Norwalk Fabr!cators. It ns purchased by TVA frc:

Ccabustion Eng.'neering.

Interi: Progress Me:bers of TVA, Norwalk Fabricators, and Cc bustien Engineering (CE) =et onsite on October 16, 1980. After a visual inspection, a conference was held, and the subject deficient welds were discussed.

Since that T.eeting, Cc:bu:.:tien Engineering Fewar Syste=s and its subtier vender, Nerwalk Fabricators of Eranferd, Connecticut, have agreed with TVA that repair of the subject deficient welds will be necessary.

TVA has informed CE that TVA is willing te perfer: the actual repair if CE will accept Ibil responsibility for repair precedures. TVA has suggested guidelines and objectives for these precedures and reserves the right to review and approve any' vendor prc:edure before it is undertaken.
