OEFECTS IN RCSF20UNT TRIP /CALI3RATICN UNITS AND PRESSURE TRANSMITTERS 10CFR50.55(e) REPCRT NO. 4 (FINAL)
Cn March 28, 1980, TVA notified NRC-CIE Region II, Inspecter R. W.
Wright, of a reportable deficiency under 10CF350.55(e) regarding defects in Rosecount trip /calibratien units and pressure transmitters being supplied by GE for use at the Hartsville Nuclear Plant. He was also notified on April 15, 1980, that the condition also exists for equip =ent being supplied for use at Phipps Eend Nuclear Plant. These deficiencies were reported to NRC-CIE Region V, Inspector R. H.
Engleken, under 10CFR21 on March 21, 1980, by GE. both verbally and by letter MFN C65-80.
This is the final report on this reportable deficiency.
Descriotien of Oeficiency General Electric was advised by Rese= cunt that sc=e of the =cdel 51000 trip / calibration units (trip units) and =cdel 1152 pressure transmitters contain defects. The trip unit is used in cenjunction with the pressure trans=itter to provide high or low trip signals.
The pressure transmitter is used for direct pressure =easure=ent trans=1ssicn or as a level senser la conjunctien with a differential pressure =easuring device.
The trap unit has a ec==cn =cde failure of two essential switches that deter =ine the trip cutput logic. The =alfunction open-circuits the 4
cittput logic, thereby providing an inoperable signal. As an exa=ple, when used as an ECCS water level trip unit, this can result in failure to trip on low wate_ level. The trip units involved are those with serial nu=bers 3001 to 4203 The transmitter has a oc==en =cde failure of a capaci* tor when the trans=itter operating te=perature is greater than 175 F, and/or when the damping potentic=eter is rotated clockwise. The failure is
=anifested as a drop in transmitter cutput current up to 10 percent of the correct current. As an exa=ple, when used as a pressure trans=itter, this can result in trans=itting reactor pressure 10 percent lower than actual. This condition applies to =cdel 1152 Rose = cunt trans=itters with "E" cutput codes and =cdel nu=bers ending in T0280, T0455, and T0400.
No trip units have been shipped to either Hartsville or Phipps Send.
These units are in control rec = panels which have not been shipped to either site.
TVA has received nu=erous 1152 trans=itters at both Hartsville and Phipps Send, a nu=ber of which are susceptible to the failure =cdes described above.
o Safety !=plicatiens The existence of the defects described above in both the 510DU trip units and the 1152 pressure trans=itters ceuld cause either inaccurate infor=ation to be trans=itted to the reactor operaters er failure to trip and autc=atically activate a safety syste=. These conditions could jeopardi=e the safe cperation of the plants.
Corrective Actions GE and/or Rose = cunt has indicated they will replace the defective switches in the =cdel 510DU trip units. Switch replace =ent will be verified by GE, San Jose, for all centrol roc = panels (the only equip =ent containing the trip units) designated for the Eartsville or Phipps Send Nuclear Plants bercre ship =ent to TVA.
GE and TVA have identified at the sites those 1152 trans=itters which could have the deficiency.
GE has issued field disposition instrue:1cns (FDI's) to the sites for the inspection of the trans=itters and replace =ent of the a=plifier cards containing the defective capaciter on affected transmitters. GE has shipped so=e of the new a=plifier cards to the sites for replace =er.t of the defective cardJ. The re=ainder of the work at the site (inspection and replace =ent) will be acce=plished by June 19, 1981. Trans=itters which have not yet been shipped to the sites will either be corrected at San Jose er at the vendcr's shop (Rose = cunt). The letter "R" will be added as a suffix to the existing serial nt=ber to indicate that the =cdification has been acce=plished for all trans=itters. TVA will continue to =cnitor receipt of equip =ent centaining the subject trans=itters to verify that the trans=1tters have been corrected (if necessary), and will correct any which are found to centain the deficiency discussed herein.
