AECM-80/254,'10/13/80 On September 12, 1980, Mississippi Power & Light Company noti-fied Mr. M. Hunt of your office of a Potentially Reportable Defi-ciency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns the failure of a tack weld on a safety related automatic air damper supplied by Pacific Air Pro-ducts Company.

We are continuing to work with the constructor to determine the reportability of this item under 10CFR50.55(e) and 10CFR21.

! Our Status Report No. 2 is attached. We expect to file our final report on this matter by July 29, 1981.

Please note that although this report was due on January 21,

Description of the Deficiency The tack weld on the shaf t of a safety related automatic air damper failed during an attempt to open the blade. The tack weld connects the shaft to the blade bracket. The dampers 1 are Q1X77F001A-A and QlX77F003-C in the Safeguard Switchgear and Battery Rooms Ventilation System (X77). These dampers

. were supplied by Pacific Air Products Company under Bechtel Specification No. 9645-M-617.1.

II. Plans for Resolving the Deficienev Mississippi Power & Light Company is working with the construc-tor to evaluate the deficiency for reportability under 10CFR 50.55(e) and 10CFR21. The following steps will be performed and tracked under Bechtel Quality Action Request (QAR) No.

F-234 to ensure that this item is adequately resolved:

1. Evaluate the deficiency for effect on safety of plant operations;
2. Determine the extent of the deficiency; and
3. Perform corrective actions, including actions to prevent recurrence.

III. Status of the Proposed Resolution The constructor is still investigating the condition. However, since our initial report, eleven other similar failures have been documented. Remedial action will be to grind out the existing weld and reveld. .This remedial-action is currently underway. Determination of cause and actions to prevent recur-rence are still in progress.

IV. Reason for Delay in Final Report Investigative efforts are not complete.

V. Final Report Date We expect to file our final report by July 29,.1981.
