ENCLOSURE SEQUOYAH NUCLEAR PLANT UNIT 2 NRC INSPECTION REPORT 328/80-22 RESPONSE TO SEVERITY LEVEL V VIOLATION 328/80-22-01 Severity Level V Violation 328/80-22-01 10CFR50.55(a), paragraph (d), as implemented by TVA's FSAR, Table 3.2-4, requires in part,

... piping which is part of the reactor coolant pressure boundary shall meet the requirements set forth in.

.. the Class I section of the USA Standard Code for Pressure Piping (USAS B31.7)..." Paragraph 1-727.4.2(c) of B31.7 states, "When components of different outside diameters are welded together, there shall be a gradual transition between the two surfaces." In addition, paragraph 1-727.4.2(d) states, "The finished surface of the weld shall merge smoothly into the component surface."

Contrary to the above, several reactor coolant pressure boundary piping welds do not blend smoothly into the adjacent valve nozzle.

Admission or Denial of Violation The unacceptable welding, as described in the subject inspection report, occurred as stated.

Reason for Violation The subject violation occurred as a result of the failure to comply with the Class I section of the USA Standard Code for Pressure Piping, i

paragraph 1-727.4.2(c) of B31.7.

Corrective Steps Which Have Been Taken and Results Achieved Repair procedures have been initiated for the unacceptable welds (2RX 00064, 00065, 00057, 00058).

Corrective Steps Which Have Been Taken to Avoid Further Noncompliance All welding inspectors have been reinstructed in the requirements pertaining to transition welds blending smoothly into adjacent base metal with a 3-to-1 taper. An additional sampling of similar valve-to-pipe transition welds will be conducted to determine if there are other welds that do not have the required 3-to-1 transition, and any that are found to be unacceptable will be repaired..

Date When Full Compliance Will Be Achieved Reinstruction for the welding inspectors has been completed, and all required weld repair will be completed by April 1, 1981.
