On August 26, 1980, Mississippi Power & Light Company notified

Mr. M. Hunt of your office of a Potentially Reportable Deficiency I
(PRD) at the Grand Gulf Nuclear Station (GGNS) construction site.
This deficiency concerned the failure of pillow block bearings in the Standby Service Water Basin Fans.
l We are continuing our investigation of this item for reporta-bility under 10CFR50.55(e) and 10CFR21. Our status report on this deficiency is attached.
" We expect to complete our evaluation and report to you on this matter by April 22, 1981.

Description of Deficiency Pillow block bearings on three of the eight Standby Service Water Cooling Tower Fans (Q1PA1C002A-A, B-A, and C-B) for GGNS Units 1 and 2 supplied by Ceramic Cooling Tower of Fort Worth, Texas, failed completely after 300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br /> of operation. This condition did not meet the Bochtel Design Specification 9645-M-015.1, which specified a minimum life expectancy of 100,000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> for radial and axial shaft load, roller bearings.
II.
Approach to Resolution of the Deficiency The following actions have been planned to resolve this deficiency:
a.
Determine the cause of the bearing failures; b.
Evaluate the deficiency to determine its affect on safe oper-ations of the GGNS. This evaluation will assume that the deficiency remains uncorrected throughout the expected life-time of the plant; c.
Determine if similar deficiencies exist, or could exist, and take appropriate actions to correct them; and d.
Perform corrective actions to include actions to preclude recurrence.
The above actions are being tracked under Bechtel Quality Action t
Request, QAR F-218, which is available for inspection at the GGNS job site.
III. Status of the Proposed Resolution The constructor and supplier are conducting evaluations to deter-mine the cause of the failure. The supplier has recommended re-placement of the damaged bearings with bearings having a different model number. Unit 1 bearings are scheduled to be replaced in January, 1981, with Unit 2 bearings to be replaced later.
Ceramic Cooling Tower has also recommended replacement of the drive shafts on the three affected fans with the drive shafts from the Unit 2 fans and that the Unit 1 drive shafts be returned to them for ev&luation.
IV.
Reasons for Final Report Delay The investigation to determine reportability has not been completed.

A final report will be available April 22, 1981.
