Date of Incident: December 29, 1980 2.
Identification of specific effluent limitation exceeded:
Daily maximum limitation of 20 mg/1.

Description of Noncompliance - Include period of noncompliance, exact date and times., If not immediately corrected, what is the anticipated time the noncompliance is expected to continue?
A sample of effluent was found to contain 37.5 mg/1.

Apparent cause of noncompliance:
Rupture of emulsion breaker filter element.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence of the nonccmplying discharge:
Filter element was replaced January 7, 1981.
The oil removal system (separator, filter, emulsion breaker and Tank T41) will be cleaned and each operator will be informed as to the expectations placed on them with regard to the oil and grease limitations of the NPDES Permit.
