The information presented is as initially received wit 1out verification or evaluation and is basically all that is known by State Agreements Program staff as of this date.

SUBJECT:
Revocation of License This is an update and close-out of Non-Routine Event No. 80-29 issued on August 22, 1980. The Office of State Programs was informed on December 16, 1980 that the Arkansas license issued to Nuclear Diagnostic Laboratories has been revoked.
The licensee had been awarded several delays on an order for a show-cause o
hearing for revocation of the license. On October 24, 1980, the State issued an enforcement letter which required the licensee to request a hearing within 30 days or face revocation of their license. To date the licensee has failed to respond and the State has revoked the license. The licensee was notified by letter on December 3,1980.
