Dear Mr. Grier:

On October 22, 1980, in accerdance with 10CFR50.55(e), we reported, verbally, to Region 1, a deficiency with 480V Nema Size 3 Contactors associated with Square D 480V motor control centers.
This letter serves as our 30-day written report of this deficiency.
Description of Deficiency Some Nema Size 3 contactors manufactured during the period of July 1977 through December 1979 have, contrary to engineering standards, had a lubricant applied to the rubber " bumpers" which are attached to the movable starter armature.
This lubricant can become tacky, causing the armature bumpers to stick to the inside of the con-tactor case.
As a result of this, the contactor may not close when required.

This deficiency was discovered by Square D and was reported to - '. '
Stone t, Webster Engineering Corp. and LILCO under 10CFR21.
A field survey of all Size 3 contactors found that fifteen (15) contactors fell within this date " window" and thus might have this defect.
One of the 15 was a Cat. II spare (installed) and ten (10) others were Cat. I spares.
The remaining four (4) are utilized for motor-operated valves, lEll*MOV037A and 037B (two reversing starters),
in the Residual Heat Removal System, a Category I Application.

Mr. Grier November 20, 1930 Page 2 Corrective Action Square D engineering has been notified and arrangements have been made to have a Square D factory representative come to our jobsite and repair all fifteen (15) contactors.
We estimate completion of these repairs by February, 1981.
Corrective Action to Prevent Recurrence We have been advised that Square D factory engineering and Quality Assurance are fully aware of this problem and have instituted cor-rective action to insure that this misapplication of lubrication to contactor bumpers does not occur in the future.
