Dear Mr. O' Leary:
By letter dated November 20, 1973, Consumers Power Company reported that a spent fuel rod had been unexpectedly found on the spent fuel pool floor. At the time the fuel rod was found, the fuel pool was being drecined in preparation for relining of the pool and it was not possible to make a positive identification of the rod. The relining of the pool has been completed, the pool refilled and inspection equipment reinstalled. The spent fuel rod has been positively identified. This letter is written to follow up the November 20, 1973 letter.
The reinstallation of the spent fuel pool periscope allowed conclusive identification of the fuel rod as DE-0289 which had been in-stared in fuel bundle E-69 Bundle E-69 was discharged from the reactor in February 1971 and subsequently shipped off-site to a reprocessor on June 20, 1973 General Electric Company verified the original location of the rod by review of QC Loading records.
The rod was apparently misplaced in January 1972. A daily work report for January 1972 via a General Electric fuel inspection showed that this rod was removed from its tie rod location, found to be damaged (upper shank broken), and allegedly returned to a cobalt corner rod location in bundle E-69 for shipment off-site to a reprocessor.
To preclude recurrence of a similar incident, vendor's fuel inspection teams are presently being required to utilize specific fuel handling procedures with sign-offs required by two individuals on any fuel rod move. Further rods that are to be shipped off-site individually for destructive examination vill be visually identified by the plant staff prior to loading into the fuel shipping cask as a do on vendor records.
