APPENDIX A NOTICE OF VIOLATION Nebraska Public Power District Docket: 50-298 Cooper Nuclear Station License: DPR-46 During an NRC inspection conducted November 1 through December 15, 1989, a violation of NRC requirements was identified. The violation involved failure to follow procedure.

In accordance with the " General Statement of Polic Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989)y and

, the violation is listed below:

Failure To Follow Procedure Title 10 CFR 50, Appendix B, Criterion V, states, in part, activities affecting quality shall be accomplished in accordance with procedures.

Plant Services Procedure 1.6, " Warehouse Marking and Tagging," Revision 5, dated June 9,1988,Section V.B.2.b.4) requires, in part, that after equipment installation "The individual perfoming the work shall be responsible for removing the HOLD Tag and placing it with the Work Item file."

Contrary to the above, after installation of the B train instrument air postfilter was completed on November 30, 1989, the hold tag on the postfilter was removed, but was not placed with the work item file.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further violations, and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
