NOTIFICATION OF DEFECT SHOREHAM NUCLEAR POWER STATION - UNIT NO. 1 LONG ISLAND LIGHTING COMPAT Stone & Webster Engineering Corporation (S&W), pursuant to the provisions of ~

10CFR21, hereby provides notification of a " defect" concern.ng the Shoreham

~

Nuclear Power Station, Unit No. 1, owned by Long Island Lighting Company (LILCO) and for which S&W provided both engineering and construction services. .

In accordance with the requirements of Section 21.21(b)(3), the following information is submitted:

1. Name and address of the individual informing the Commission:

2. Identification of the facility, or the basic component supplied for such facility, which contains a defect:

Shoreham Nuclear Power Station, Unit No. '1" Square D Company 480V Motor Control Centers - NEMA Size-3 starters with catalog numbers 8536SE01, 8736SE02, 8502SE0*

manufactured between July of 1977 through December ei 1979.

3. Identification of firm constructing facility or su;, plying basic component which contains a defect:

4. Nature of the defect and the safety hazard which could be created by such defect:

There exists a departure from the technical requirements of a procurement document in that the starter or contactor. could fail to operate as specified.

The subject equipment has been delivered to a purchaser and qualifies as basic componer;s.

The problem involves application of lubricant to the rubber bumpers attached to the covable starter armature which in some instances may become tacky, causing armature bumpers to stick to the inside of the starter case. As a result, the starter or contactor may not close when required.

The contactors are required under design basis events to mitigate the consequences of accidents. If the defective contactors (after a LOCA) do not open the low pressure coolant injection (LPCI) outboard containment isolation valves upon demand, then it is possible to lose both redundant paths for LPCI. Degradation of such essential safety-related equipment represents a major reduction in the degree of protection provided to public health and safety and could create a substantial safety hazard. -

December 2, 1980

6. In the case of a basic component which contains a defect, the number and location of all such components in use at, supplied for, or being supplied for facilities subject to the regulation:

Shoreham Nuclear Power Station - three possible defective units (IR24* MCC 1114 - Cubicle 4IL, IR24* MCC 111X -

Cubicle ICI, IR24* MCC 112Y - Cubicle 2CI).

Our investigation to date has shown no defect exists on any i other S&W plants. Should this information change, this report will be supplemented and appropriate advice will be given to purchasers or licensees.

7. The corrective action which has been, is being, or will be taken; name of individual or organization responsible; and length of time I

All defective units identified by S&W Field QC at Shoreham will be replaced with new units. The anticipated comple?. ion.

NRC has been previously advised of this problem by LILCO Notification (SNRC-516) on November 20, 1980, in accordance with 10CFR50.55(e).
