The letter referenced above provided the results of a special safety inspection conducted by Hs. D. R. Calhoun of your office during September 25 through October 5, 1989 of activities at Zion Station.

During the course of l

this inspection, certain activities appeared to be in noncompliance with NRC l

requirements.

ATTACHNENT Violation:

I Technical Specification 3.7.2.c requires that with the TDAFH pumps inoperable, two operable flow paths shall be established from the MDAFH pumps to all steam

veten within eight hours.

Contrary to the above, Unit 1 opet6ted at pnwer in the run mode from 6:40 P.M.

on July 23, 1989, to 11:48 A.M. on July 24, 1989, which exceeds the time allotted by Technical Specification by nine hours and eight minutes, without establishing two operable flow paths from the MOAFH pumps to the steam generators.

This is a Severity Level IV violation (Supplement I).

l-Corrective Action Taken and Results Achieved:

I The flow rates to each of the four steam generators from the IB Auxiliary Feed Water pump (AFH) were immediately adjusted to provide the required flow of 105 l

GPH.

The 1C AFH pump was also run to verify the availability of both the AFH pump and the second flow path.

Corf.e_ctive Action To Be Taken To Avoid Further Violat10D:

Station procedure 501-10 " Auxiliary Feed Hater" is used as guidance to direct valve manipulation to split the AFH header. The applicable section of this procedure has been revised to include the verification of the flow rates at 105 GPM as provided by the 1B AFH pump after AFH header alignment.

Additionally, (Periodic Test) PT-7, " Auxiliary Feed Hater System Checks and Tests" has been revised to incorporate a similar requirement, l
