Notice of Violation St. Anthony' Hospital License No. 12-00351-03 Based on the inspection conducted on April 13, 1981, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Items 1 through 4 are infractions, and item 5 is a deficiency.
1.
10 CFR 20.101(a) limits the whole body exposure of an individual in a restricted area to 1.25 rems per calendar quarter, except as pro-e vided by 10 CFR 20.101(b). Paragraph (b) allows a whole body exposure of 3.0 rems per calendar quarter provided certain specified conditions are met.

second calendar quarter of 1978 and the conditions of paragraph (b) j were not met.

10 CFR 20.405(a) requires that you submit within 30 days, a report to the Commission concerning' each exposure to radiation in excess of any applicable limit in Part 20 or in your license.
10 CFR 19.13(a) requires that this report be submitted to the individual exposed.
Contrary to these requiements, as of April 13, 1981, you failed to report to the Commission, and to the individual exposed, the exposure

License Condition 21C requires the licensee to establish written procedures for personnel performing tests to detect and quantify

molybdenum-99 contamination. These procedures shall include all necessary calculations and steps to be taken if activities of molybdenum-99 in excess of the limits specified are detected.
I Contrary to the above requirement, it was learned through statements of licensee representatives that written procedures were not available.

License Condition No. 18 requires that licensed material be possessed and used in ccordance with the statements, representations, and pro-l cedures contained in application dated November 28, 1978.
Item 10 of the application states the procedures described in Appendix D, Section 2 of the NRC Medical Licensing Guide will be used for cali-bration of the dose calibrator. Appendix D, Section 2 requires daily constancy checks of the dose calibrator and records maintained.

Specifically, only weekly constancy checks were performed.

-License Condition No. 18 requires that licensed material be. possessed and used in accordance with statements, representations and procedures contained in application dated November 28, 1976.
Item 14 of this application requires that' records be made on all package surveys of i
incoming radioactive material.
Contrary to the above requirement, it was learned through statements of licensee representatives and the absence of records, that this re-quirement has not been met since the effective date of this license December 20, 1978.
