APPENDIX A NOTICE OF VIOLATION South Carolina Electric and Gas Company Docket No. 50-395 Summer License No. CPPR-94 As a result of the inspection conducted on February 3-5, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.
10 CFR 50, Appendix B, Criterion V, as implemented by paragraph 17.1.5 of the FSAR, requires in part, Activities affecting quality shall be pre-scribed by documented instructions, procedures, drawings...and shall be accomplished in accordance with these instructions, procedures, and drawings.
Paragraph 4.1 and Appendix D, paragraphs B.9 and B.10, of QCP-VII-02, the applicable procedure for control of fabrication and instal-lation inspections, requires that the craft foreman and QC inspector sign and date operations and inspections on the " Packet Instruction Sheet" as they are completed.
Contrary to the above, activities affecting quality were not accomplished in accordance with documented procedures in that for replacing the bellows on steam generator blowdown penetration 326P and installation of electrical penetrations XRP0020, XRP0043 and XRR0055, many operations on the " Packet Instruction Sheets" were not signed and dated by the craft foreman and QC inspector as the operations were. completed.
This is a Severity Level VI. Violation (Supplement II.F).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
