' s Appendix A NOTICE OF VIOLATION Dairyland Power Cooperative Docket No. 50-409 As a result of the inspection :onducted on November 1-30, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

LACBkB Technical Specification 4.2.5.7 states:
"Except during plant startup and shutdown, the main steam bypass valve
- control shall be set to begin opening the bypass valve at an indicated pressure corresponding to a turbine inlet pressure not more than 15 psi above its normal value."
Contrary to the above, on September 15, 1980, with the reactor at steady state power and on September 16, 1980, with the reactor at steady state power, the resident inspectors noted that the main steam bypass valve control was set to begin opening the bypass valve at 19 psi, and 17.5 psi, respectively.

LACBkT Technical Specification 3.8.1 states:
" Written procedures and administrative policies shall be established, implemented and maintained.
Contrary to the above, licensee personnel failed to properly complete the shift turnover log required by Administrative Control Procedure (ACP) 2.3 on November 10, 11, 12, 17, 18, 25 and 27, 1980.
This is a Severity Level V violation. (Supplement I.E.)

LACBkB Technical Specification 3.8.2 states:
"Each procedure and administrative policy of 3.8.1 above, and changes thereto, shall be reviewed by the ORC and approved by the Plant Superintendent prior to implementation and every two years thereafter."
Contrary to the above, on November 14, 1980, the licensee personnel were performing refueling operations utilizing procedures in Volume VI of the LACBkV Operating Manual that had not received the required two year review.
This is a Severity Level IV violation. (Supplement I.D.1)

LACBWR Technical Specification 3.8.1 states:
" Written procedures and administrative policies shall be established, implemented and maintained.
Contrary to the above, licensee personnel failed to monitor source range indication during fuel movement in the reactor vessel, on November 18 and 19, 1980, as required by Volume VI of the LACBWR Operating Manual and Fuel Transfer Procedure No. FC-31-80-1, Procedure No. 2.
Contrary to the above, the Control Room Accountability watch failed to initial any entries made on his watch for fuel transfers on swing shift November 18, 1980.
Contrary to the above, the briefing book for fuel movement, utilized by operators to verify a prerequisite to fuel movement as required by Volume VI of the LACBWR Operating Manual, had erroneous information in that it stated the minimum water level in the Fuel Element Storage Well above fuel racks to be two feet while the LACBWR Technical Speci-fication is 16 feet above the fuel.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this of fice within twenty-five days of the date of this Notice a written statement or explanation in reply, including for cach item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or af fi rmation.
