Gentlemen:
Prompted by your letter dated 26 November 1980, we have resiewed our file copy of Technical Operations, Inc. Certificate of Compliance No. 9032, Revision No. 2, for the Model No. 650 shipping package which was trans-mitted to us by you on 17 September 1979.
We find that the drawings referred to under 5(a)(3) of the Certificate did not acco=pany the Certificate.
10CFR71.12(b)(1)(1) states that we should have copies of all documents referred to in the certificate, which in this case would be the applicable drawings.
It was also noted that we were listed on the 17 September 1979 transmittal as users of this package as Omaha Testing Laboratories, Inc. On 2 October

1979, we had advised that the company name had been changed to Omaha Non-l destructive and Metallurgical Testing, Inc. and such name change had been l
revised on our applicable radioactive material licenses.
To ensure compliance with 10CFR71.12(b) we are requesting copies of the l
applicable drawings and other pertinent documents. Also, the company name l
should reflect the above change and correspondence should be directed to I
the undersigned.
