Dear Mr. Schwensfeir:

The Jordan Hcspital hereby agrees to enter into a formal agreement with Texas Instruments to accept and provide medical services to any victims centaminated by radiation at your plant. For our mutual protection and to provide the best possible service to radiated victims transferred to us, we ask that you carry out tha folicwing prccedure: (1) m tify The Jordan Hospital Emergency Department supervisor, as soon as a possibility exists that victims are to be transferred to The Jordan Mcspital (617 - 746-2000, Ext. 311 or 312). (2) Decentaminate any victims as thoroughly as possible. Clothing should be removed, if possible, and retained at the Texas Instruments plant for later examination. (3) Victims should be brought to the Emergency Department entrance of Sandwicn Street *(see enclosed diagram). The Jordan Hospital reserves the right to cancel this agreement sh3uld any conditions at either institution change significantly (i.e., changes in location, changes in safety practices and procedures, changes in the type (s) of adioactive material handled, etc.). The altering party is obligated to l notify the other party as to the changes. Othenvise, the agreement is void. ) If this agreement is acceptable to Texas Instruments, please sign and l return one copy of tnis letter to Nalter A.
