Dear Mr. Grier:
Pursuant to the Requirements of Technical Specification 3.7.8, the following report is submitted as Special Report 50-29/78-01, Loss of Environmental Radiation Monitor.
i Ciretrastance of Violation:
On March 31, 1978 when the Monthly Environmantal Thermoluminescent Dosimeters were collected, it*was noted that the monitor at Station 12 was missing. This resulted in the collection of 22 monitors rather than 23 as required by Table 4.7-4 of our Technical Specifications.
A thorough search of the area near the Station failed to locate the monitor.
Prevention of Reoccurrence:
In order to attain optimum perfomance from these monitors, it is necessary that they be as completely unshielded as possible. This requirement limits the extent to which they may be fastened at the Station.
Ilistorically the loss of radiation monitors has not been a problem. Therefore, we plan no preventive action.
