Interim Report on Problems in Start-Up Operations.

Dear Mr. Keppler:

On June 27, 1980, Mr. H.A. Walker of Detroit Edison's Quality Assurance Department telephoned Mr. T. Van Dell of NRC-Region III regarding problems found by the Edison Start-Up Organization during an internal performance review of their operations.

The internal performance review was performed by the Start-Up Organization on March 28, 1980, to determine the degree of compliance with the require-ments of the Start-Up Manual Appendix 4.2 " Control and Storage of Electri-cal Group Measuring and Test Equipment". Deficiencies were found with the operations of the Corporate Division Services - Meter Services Standards Laboratory located at the Warren Service Center which was performing cali-bration for Start-Up, and with records initiated during testing operations.

The lab appeared to be operating without adequate procedures and did not, in all cases, have the records to substantiate the calibration and accuracy of equipment used during Start-Up Testing. Deficiencies found with record-keeping practices of the Start-Up Technicians indicated a lack of suffi-cient training to the requirements of the Checkout and Initial Operations procedures.

Based upon the deficiencies found, all electrical testing of QA Level I equipment was suspended effective March 28, 1980. Presently, test equip-ment is being sent to Bailey Meter in Cleveland and G.E. in Detroit for calibration. Use of the Edison lab will be resumed when written procedures are developed and implemented.

The corrective actions necessary to bring this program into compliance with controlling procedures and to re-evaluate tests previously performed are being complied by the Edison Start-Up Organization. A final report on this matter is scheduled to be submitted to you by October 27, 1980.
