Limiting Condition for Operation (LCO) 3.3.3.7 of the Trojan Technical Specifications (TTS) requires submittal of a Special Report within 30 days if one or more of the fire detection instruments shown in Table 3.3-10 are inoperable for more that fourteen days.
On May 7, 1989, KZIU-6-6 4, detector U749 was out of service for more than the fourteen day limitation because of a misplaced Maintenance l
Request.
Notification by Special Report was delayed because of failure to track the Maintenance Request completion progress.
I A new Maintenance Request was submitted and the affected component was restored to operability on September 25, 1989.
Additional administrative controls are being implemented to improve the tracking and reporting of fire detector outages.
The compensatory measures of Technical Specifications-3.3.3.7 (establishing an hourly fire patrol to inspect the affected area) was l
established as required.
