QUALITY A$$URANCE PROGRAM:

The purpose of this subvsttal is to have a Quality Assurance progrem i
to assure safety in shipping radioactive materials of Iridium 192 l
and Cobalt 60 in the usage of industrial radiography.
This Quality Assurance Program will assure safety practices in the following manner:
1.
It.spection of received shipping containers for damage and/or corrosion, l
2.
Re:ording of received shipping containers on prescribed i
forms.
L 3.
Checking for seal of received shipping containers.
4.
Inspection of container being shipped for damage and/or corrosion.
l 5.
Recording of container being shipped.
6.
Checking for sealed condition of container being shipped.
7.
Reporting of shipping containers being received or shipped to persons of authority as illustrated in organizational.
i 8.
Pecording of handling procedure for radiation monitoring in accordance with TIA receiving / shipping form.
9.
Audits of TIA management to assure Quality Assurance records are maintained and proper inspection of shipping containers are performed.

RECEIVING SHIPMENT:
Personnel receiving radioactive material vill inmediately make f
survey of radiation with a properly calibrated survey meter and I
record all required informa. ton on rectiving form. Personnel will then inspect shipping container for damage and/or corrosion visually and check container seal for proper condition.
CONTAINERS BEING SHIPPED:
Personnel preparing radioactive material for shipping will visually inspect container (s) for damage and/or corrosion.
If l
container is seen to be in good condition, personnel preparing shipment will seal container making sure all labeling and shipping papers are in order making entry oa the container form and other chipping documents to insure proper condition and handling

procedure.
DAMAGED CONTAINERS:

Personnel either receiving or preparing containers for shipment will notify the proper TIA management if any condition is found that could make further use of the shipping container a possible safety hazard. Any container found to be in an unsafe condition would be discontinued for use.
t AUDITS:

Management Personnel will audit shipping containers inspection forms within thirty days of shipping activity to insure proper procedure was used.
