This refers to the routine safety inspection conducted by Mr. John R. Madera of this office on June 1,1981, of activities at St. Luke's Medical Center authorized by NRC Byproduct Material License No. 14-18721-01 and to the discussion of our findings with Mr. Ken Gardner, Senior Vice President, J. J. Coebel, M.D. sad Roger Ericksen, Chief Nuclear Medicine Technologist at the conclusion of the inspection.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your licence. The inspe:-
tion consisted of a selective..aination of procedures and representative recordo, observations, independent measurements., and interviews with personnel.
Dur.ng this inspection, certain of yeur activities appeared to be in non-
- ompliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
We will gladly discuss any questions you have concerning this inspection.
