NOTICE OF VIOLATION Soiltest, Incorporated License No. 48-15426-01 As a result of the inspection conducted on June 3, 1981, and in accordance with the Interim Enfdecement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
License Condition No. 15 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in. qplication dated January 17, 1978.
Item No. 11 of the above referenced application states survey meters used in the radiation safety 'rogram will be calibrated at least every six months.
Cratrary to the above, it was determined through statements by licensee representatives that this requirement was not met.
Specifically, Eberline Instruments, Model E-400, Serial No. 2596, and Precision Radiation Instruments, Model III, Serial No.11H-7564 were last calibrated on April 23, 1979. This period is in excess of the six month calibration requirement.
This is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10"CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your' response time for good cause shown.
