l p (,,- UNEXP'ECTED INCREASE IN SRM COUNT RATE PLANT CONDITION AND EVENT

SUMMARY

On April 1,1981 at 0700 hours, control rod functional /subcritical checks and friction testing were being performed. The reactor mode switch was in the " Refuel" position; Control rod 42-27 testing was-in progress with Source Range Monitor (SRM) channel 22 and channel 23 selected on the SRM recorder. The recorder speed was one(1) inch per minute. Attachment I shows the relative location of the control rod being tested and the selected SRM's. In accordance with OP 690C, control rod 42-27 was single-notched from position 00 to position 43 with no appreciable increase in the SRM count rates (see attachment II). The rod was then inserted from position 48 to position 00 with " Emergency in". switch and timed at 38 seconds. Several attempts at continuous withdrawal were stopped due to the need for control rod speed adjustments. On the final atte. apt at withdrawal,.the red passed position 02, and at position 03, " Emergency In" was used to retuin the rod to po:,ition 00. At this point, it was noted that the SRM channel 22 count rate had more than doubled and the SRM channel 25 count rate also had a significant increase (see attachment III). Control rod testing was stopped pending investigation. INVESTIGATION AND ANALYSIS Although it was felt that the increase in SRM count rates was not attributable to a corresponding increase in neutron count rates, information concerning the incident was forwarded to NUSCo's Reactor Analysis Group to perform calculations of the reactivity increase for rapid reactivity insertion based on the available data. General Electric was also contacted to perform independent calculations. A dynamic analysis performed by NUSCo indicated a reactivity increase of 0.09% a K/K; a static analysis performed by General Electric indicated a reactivity increase of 0.008% A K/K. The reporting limit specified in Technical Specifications for unplanned raactivity insertion is 0.5% A, /K. K In parallel with the reactor physics' analyses, I&C and Operations performed testing to determine whether the indication of count rate increase was due to an electronics problem. SRM functional checks were satisfactorily completed, indicating a neutron count-to-noise ratio of greater than 3:1. In addition, SRM discrimator settings were checked on SRM channels 22 and 23 te determine whether the SRM response was caused by electronic noise due to excessively high SRM sensitivity. SRM sens'tivity had been increased during fuel loading to achieve SRM operability when problems were encountered with the portable dunking chambers. This testing indicated that the SRM channel '2 discriminator setting was higher than normal; the discriminator was~ reset from 9.383 volts to 8.99 volts and response was satisfactorily tested. Discriminator and high voltage curves for SRM channels 22 and 23 are attached (Attachment IV-1 thru IV-4). O G G.

PORC REV'2V lhe PORC reviewed the incident cnd follow up analysis. It was decided that a symmetric core. location in which fuel cell loading from a core physics vies point is comparable to location 42-27 should be tested prior to retest of 42-27. Location 10-27 was identified to be tested for this purpose. Additional details are listed iri the minutes of PORC meeting PORC-I-81-52. FOLLOW-UP AND RETEST The f unctional/subcritical check for Control Rod 10-27 was successfully complbfed. An increase of the neutron count rate as indicated by SRM channel 24 was noted, as expected, due to the relative location of the detector to the rod being tested (see Attachment I). Retest of Control Rod 42-27 was then successfully completed. l It was concludtd that the increased count rate during initial testing of Control Rod 42-27 was caused by electronic interference induced by other re,ays cr welding in the area of the SRM channel 22 circuitry and intensified by the increase sensitivity of the SRM channel. Prior testing had indicated that induced noise signals show characteristics similar to neutron period decay curves when the chart recorder is placed in fast speed..
