Mr. Dean W. Nelson Administrator 410 Dewy Street Wisconsin Rapids, Wisconsin 54494 Gentlemen:
This refers to the routine safety inspection conducted by Mr. J. L. Lynch and Dr. L. W. Shatterly of this office on June 5,1981, of activities authorized by NRC Byproduct Materials License No. 48-12878-01 and to the discussion of our findings with A. M. Lucas, M.D. and other members of your staff at the conclusion of the inspection.
The inspection was aa examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspection consisted of a selective examination of procedures and representative records, observations, independent neasurements, and interviews with personnel.
In addition to the above areas, t'h'e inspectors examined actions described

in your letter dated November 3, 1978, signed by Mr. D. J. Eymans, regarding corrective action taken on apparent items of noncompliance found during our September 12, 1978, inspection of License No. 48-12878-01.
We have no further questions regarding these matters.
During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
We will gladly discuss any questions you have concerning this inspection.
