Appendix A NOTICE OF VIOLATION Thorek Hospital and Medical Center License No. 12-12418-01 License No. 12-12418-02 As a result of the inspection conducted on May 1, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

License No. 12-12418-02 License Condition No. 27(c) requires that the electrical interlocks on the doors of the cobalt-oO room and X-Ray Rocus B and D, and the audible and visual signals in X-Ray Rooms B and D, shall be tested for proper operation at least once every six (6) months. Records of test results shall be maintained for inspection by the Commission.

Contrary to the above requirement, it was determined through statements oy licensee reprt. entatives and the NRC inspectors review of records that this condition is not being met.

Specifically, records of electrical interlock tests have not been maintained to the date of this inspection.

This is a Severity Level VI violation (Supplement VII).

License No. 12-12418-01 2.

License Condition No. 17 requires that licensed material be possessed and used in accordance with statements, representation, and procedures contained in application dated October 7, 1977.

The above referenced application states, the procedures described in Appendix B of the draft " Guide for Preparation of Applications for Medical Programs" dated February 1976, will be followed.

records that this condition is not being met.

Specifically, the Medical Isotopes Committee met on September 27, 1979, October 14, 1980, and March 4, 1981. These intervals exceed the quarterly requirement.

This is a Severity Level V violation (Supplement VII).

Appendix A statement or explanation in reply, including for each item of noncompliance:

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or af firmation.
