Public Service Company of Colorado Fort St. Vrain Docket No. 50-267 License No. DPR-34 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted during the period of January 1 - January 31, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

Technical Specification 7.4.C.3 requires that Temporary Changes may be made to procedures provided "the change is documented, reviewed by the PORC and approved by the appropriate manager within 14 days of implementa-tion."

Contrary to the above temporary changes are made to eight (8) procedures that were not reviewed by PORC within the specified 14-day time period.

This is a Severity Level IV Violation.

(Supplement I.D.3) 2.

Technical Specification 7.4.a, Procedures, Administrative Controls, states in part that " written procedures shall be established, implemented, and maintained...."

Procedure S0P 11, System Operating Procedure 11, Prestressed Concrete Reactor Vessel, Section 2.1, Prerequisites to Operation, states in part that "..

2.1.5 Valve Lineup Check List completed - refer to Appendix II."

Procedure S0P 11, Appendix II, Prestressed Concrete Reactor Vessel Valve Lineup Check List, requires in part that valve V-1110-57, Valve Stem Gland Packing Vent, be "... Open (Locked)".

Contrary to the above, on January 22,1981, at 11:30 a.m., it was determined by the inspector that valve V-1110-57, Valve Stem Gland Packing Vent, was not locked.

This is a Severity Level V Violation.

Pursuant to the provisions of 10 CFR 2.20, Public Service Company of Colorado is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including (1) the corrective steps which have been taken and the results achieved; (2) corrective steps will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
