1.0 Introduction This is issued as an interim report on the RVAC related problems at the Limerick Generating Station. It has been determined that the HVAC Subcontractor has fabricated and erected ductwork and duct hangers which are not in compliance with certain design detail drawings and specification requirements.

1 The problems are related to the structural and functional aspects of the HVAC ducts and supports in the Primary Containment, Reactor Enclosure, and Control Room complex.

It has not yet been detereined chat failure to detect any er 411 of these problems would have affected the safety of the plant.

2.0 Descriotion of Problems 2.1 Description.. , .

The problems can be categorized as follows:

2.1.1 Numerous HVAC subcontractor hanger sketches were, to varying degrees, not in compliance with the contractor design detail drawings.

2.1.2 Several HVAC duct sections and fittings were fabricated with deviations from contractor supplied fabrication design details.

2.1.3 Several HVAC ducts and fittings were veld-with welds which deviated from the contra..;r sup) lied weld detail design.

3.0 Analysis of Safety Implications It has not yet been determined that feilure to detect and repair these various proble=s'would have affected the safety of

, t~ns plant. Complete and detailed eyaluations are being conducted to determine the safety implications.of each

. noncompliance. Based on the engineering evaluations completed to date, it is anticipated that a majority.of noncompliances will be dispositioned "use as-is".

4.0 Corrective Action Taken 4;l A fabrication'and installation " hold" was imposed by the subcontractor at the site and at the Pittsburgh fabrication facility.

4.2 The installation hold at the site was subsequently released contingent o r- 100% inspection of all fabricated 3;

unerected material.

4.3 An audit of the Subcontractur's program is planned prior to the release for fabrication at the Pittsburgh fabrication facility.

4.4 All noncompliances are presently being evaluated. Some have been found technically acceptable on a one-ti=e basis; others have been found acceptable on a generic basis. For still others, the Subcontractor has been directed to repair or rework the nonconforming item to meet the drawing requirements. The remainder of the noncompliances require further evaluation.

4.5 The cause of the noncompliances is still under evaluation.

5.0 conclusion 5.1 As a result of the plan of action now being implemented, the safety and'the structural and functional integrity of the HVAC ducts-and supports will~not be compromised.

, 5.2: It-is expected that it will take approximately six months to fully evaluate all the nonconforming items. Submittal of the. final report will be by September 30, 1981.
