DATE DESCRIPTION Oct. 1980 Attended a one week (40 hr.) requalification program at Vepco's Surry Simulator.
This con-sisted of a review of significant LER's, oper-ating experience, and the completion of re-quired reactivity manipulations as per Salem's Licensed Operator Requalification Procedure.
Sep. 1980 Attended a two day (16 hr.) Recognizing /
Mitigating Core Damage course conducted by the training staff.
The course covered those topics outlined in Enclosure 3 of the Mar. 28, 1980 Denton letter.
Oct. 1979 Attended a one week (40 hr.) requalification program at Consolidated Edison's Indian Point Simulator.
Goals and accomplishments comparable to those described in Surry simulator entry above.
Additionally:

Have successfully completed all phases of the Salem Licensed Operator Requalification Program currently in effect.,

Successfully completed the salem Unit 1/2 Differences Course.
3.
Successfully completed the Salem TMI/79-06 Training Course.
