Improper Heat Treatment of A490 Bolts from Southern Bolt Company
Dear Mr. O'Reilly:
On February 17, 1981, Mr. Virgil Brownlee of Region II, was advised by telephone of a potential substantial safety hazard as defined by 10CFR21.
This item is now being reported under the requirements of 10CFR50.55(e) as a substantial safety hazard.
This item involves thirty-five (35) A-490 bolts, furnished by Southern Bolt Company, which may have received improper heat treatment.
Details are given in the attachment.
This is considered a final report on this matter, with final corrective action anticipated to be completed-4 May 15, 1981.

Indentification of Basic Component:
35 Structural Bolts - 1 " X 5" ASTM A490 3.
Identification of Firm Supplying Component:
Southern Bolt Corporation Post Office Box 7196 Shreveport, Louisiana 71107 4.
Nature of Defect and Safety Hazard Which Could Be Created:
The item in question consists of one lot of 35 structural bolts, ik" X 5" ASTM A490, furnished by the referenced cupplier.
The supplier has advised that these bolts were all included in heat number 69784 which may have received improper heat treatment.
This item has been documented by Site Nonconformance Notice 1299C, and dispositioned to have the bolts changed out for bolts fully certified to meet all requirements.
The vendor has advised NRC Region IV (Mr. Barnes) of this condition.
The bolts are all utilized on site in the assembly of the. pressurizer upper lateral support frame which provides support for the vessel against applied horizontal forces.
Although highly unlikely, it could be postulated that in a seismic event, some of these bolts, if lacking adequate strength due to improper heat treatment, could fail.
Depending on the number of failed bolts and the magnitude and. direction of the applied forces, it could be postulated that the lateral support frame would fail, which in turn could be postulated to lead to a LOCA.
Therefore, this is conservatively

being considered a substantial safety hazard and repo.-ted as such.
As noted, these bolts will be replaced by fully qualified bolts.

Date Information rf Defect Was Obtained:
February 1 1981.

Number and Location of Defect:
35 bolts, all used in assembly of the pressurizer dpper lateral support frame.

Cause and Corrective Action:
The$ problem originated in the supplier's heat Mreatment plant and occurred following a shift change.
When the supplier's new shift operator began work, he appareitly failed to load all of the bolts, that had buen normalized and set aside to air cool, back into the furnace to receive the remaining required heat
. As a. result, the bolts, which had the same physical treatment.
appearance (i.e., covered with heat treatment scale) were removed from the heat treatment area without all of the bolts being 1
properly heat treated.-
The supplier has taken action to account for all of the bolts
.which apparently were only normalized and to repla;_ chem accordingly.
The 35 bolts supplied to South Carolina Electric and Gas Company are still suspect, but will be replaced by bolts verified as having proper heat treatment.
Also, the supplier t
has assessed the QA programmatic aspects. of this problem and
- has taken action to preclude recurrence.
These actions include
. the stamping of test samples with material identification prior.
to the transmitting of these samples from the heat treating hold area 'to - the metallurgical' laboratory -for testing.
Also, in order

.to assure th'at the proper heat treatment has been performed, i
3-heat treatment records will be reviewed by the metallurgical laboratory and QA prior to the QA acceptance of the tesc results and release of GA Hold Point.
