RE: Deficiency in Execution of Operator Requalification Facility License R Docket 50-83
Dear Mr. Miller:
Pursuant to our Technical Specifications we are hereby reporting significant deficiencies noted in the execution of our 1980-1981 Reactor Operator Requal '
ification Program, established in compliance with 10 CFR 55 Appendix A.
The deficiencies, which were originally considered to be minor by the Reactor Administration, were first pointed out by the Annual In-depth Review conducted by the Reactor Safety Review Subcommittee and then fully resolved and analyzed by the Reactor Administration. The deficiencies are both in the record-keep-ing area and lack of completion of scheduled training and exam sections of the program.
- 1. Summary of Deficiencies
- a. No record of operator walk-throughs due February 1980 (only G.R. Dalton walk-through is recorded).
- b. No record of lecture on Normal / Abnormal Procedures to be given in May 1980.
- c. No lecture given on Tech Specs due September 1980.
- d. No exam given by 10/27/80 on the Reactor Instrumentation, Control and Protection System (H. Gogun lecture)*
- e. Annual report review and discussion not conducted on November 1980 as scheduled.
- f. No exam given by December 18, 1980 on Normal and Abnormal Procedures (N.J. Diaz lecture).*

- N.J. Diaz to J. Miller page two March 9, 1981 conduction of the Requal was made, the lack of a full-time person dedicated exclusively to the administration of the facility (while re-licensing was being done)resulted in the noted deficiencies.
It should be pointed out that by the nature of our operations, where educa-tional/ training and research programs are conducted year-round, the UFTR operators are routinely "requalified" and therefore no significant deficiency in personnel performance should result from this occurrence.
- 3. Actions Taken to Prevent Recurrence
- a. A Senior Reactor Operator has been appointed Acting Assistant Reactor Manager and written instructions given to establish the priority of the Requal Program.
- b. All items not executed in a timely manner will be completed by April 30, 1981.
- c. A status board on the Requal Program will be established and the required action incorporated into the UFTR monthly report by April 30, 1981. No slippages of the schedules will be permitted unless fully warranted by special conditions and a new schedule set to guarantee compliance with
.the program.
The Requal Program shall be well fit within our schedule to assure full compliance with established requirements.
