Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-237 As a result of the inspection conducted on November 15 - December 31, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
Technical Specification 6.2.A requires, that detailed written procedures including applicable checkoff lists covering normal startup of the reactor and other systems shall be prepared, approved, and adhered to.
The first step of Section 3 of procedure DGP 1-1, the unit startup procedure, states, "during pressurization of the reactor, the pressure regulator setpoint shall be maintained approximately 50 psi greater than reactor pressure."
Contrary to the above, on December 4,1980, the reactor was pressurized to approximately 250 psig in the course of a normal unit startup with the pressure regulator setpoint left at its low setpoint (rs/150 psig),
resulting in a reactor scram.
This is a Severity Level VI violation.
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
