IDENTIFICATION OF OCCURRENCE - EMERGENCY DIESEL GENERAPOR 1 TEST FAILURE efs. CONDITIONS PRIOR TO OCCURRENCE, PLANT IN COLD SHUZ STATION POWG PROVIDED VI A 138 KV LINE. 46 KV LINE AVAILABLE AS A BACKUP.

6 DESCRIPTION OF OCCURRENCE - FAILURE OF EMBCENCY DIESEL GEN-ERATOR TO START N0ttMALLY DURING ROUTINE WEEKLY TEST.

7 DESIGNATION OF APPARENT CAUSE OF OCCURRENCE - PRELIMINARY ANALYSIS SHOWS THE PR03L*EM TO BE ASSOCI ATED WITH PROVIDING AN ADED UA TE FUEL SUPPLY TO THE DIES EL ENGINE.

ANALYSIS OF OCCURRENCE - DMRIENCED DIFFICULTY IN STARTING EMERGENCY DI ESEL GENE ATOR DURING NORMAL WEEKLY TEST. THE DIESEL WENT THROUGH APPROXIMATELY 12 CR ANK OPE ATIONS BEFORE STARTING AND IT WAS ALSO NECESSARY TO OPERATE TdE HAND PRI-MARY PUMP TO MAKE IT START. ORIGINALLY, FAILURE OCCURRED AT 1100 HOURS ON 4-10-75. RETESTS VERE CONDUCTED AT 1524 AND 2000 ON 4-10-75 AND 0818 ON 4-11-75. ALL THR EE R ETESTS WER E SUCCESSFUL AND,THERE VAS NO RECURRENCE OF THE ORIGINAL INGI-D EN T.

CORRECTIVE ACTION - INVESTIGATION IS CONTINUING AND FURTHER ACTION SILL BE REPORTED AT A LATER DATE.
