Katherine Shaw Bethea Hospital License No. 12-17622-01 ATTN:

Mr. John Tatum Administrator 403 East First Street Dixon, IL 61021 Gentlemen:

This refers to the routine safety inspection conducted by Dr. L. W. Shatterly of this office on April 16, 1981, of activities authorized by NRC Byproduct Material License No. 12-17622-01 and to the discussion of our find.ings with Jeffrey Wexler, M.D., and other selected members of your staff at the con-clusion of the inspection.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

In addition to the above area, the inspector examined actions described in letter dated August 23, 1978, signed by Dr. Irma Almassy, regarding corrective action taken on apparent items of noncompliance found during our August 10, 1978 inspection of License No. 12-17622-01. We have no further questions regarding these matters.

During this inspection, certain of your activities appeared to be in non-compliance with NRC requirements, as specified in enclosed Appendix A.

A written response, submitted under oath or affirmation, is required.
