Dear Mr. Madsen,
This letter is in response to the Notice of Violation received as a result of inspections conducted at this facility during the period February 1 to February 28, 1981.
Numbering is consistent with that in the Notice of Violation.
- 1. (a) Liquid waste releases were immediately suspended until the discharge path was modified, preventing further violations.
(b) A by-pass was installed around the oil separator, to be used for liquid releases, and procedures revised to provide for a flush of the system with clean water at the release rate, on completion of i
the release.
Releases subsequent to these modifications have all shown the modifications to be effective.
(c) We are now in compliance.

- 2. (a) All personnel involved, including the working foreman, have been reprimanded for their failure to properly exit an R.W.P. area.
(b) Working Foremen and Supervisors have been instructed to emphasize RWP requirements in j ob briefings prior to job commencement and j
to continue such briefings throughout the job.
(c) We are now in compliance.
