As was indicated in References (a) ano (b), Commonwealth l

Edison has undertaken the review of a deficiency involving insulator cracks on the epoxy module portion of the Amphenol Sans electrical penetrations on LaSalle County Unit 2.

The review has been l

completeo, and rework required to resolve the deficiency defined.

The basis for resolution is discussed below.

1.

Pre-crimped Penetration Modules The rework of the pre-crimp penetration modules has been completed in accordance with the manufacturer's procedure.

Details of the repair were explaineo in Reference (b).

The penetrations were received back at I

the site on April 10, 1981 and installation is in progress.

2.

Post-crimp Penetration Modules The rework of the post-crimp modules

.5 presently underway at the manufacturer's facility.

The repair procedure consists of two methods to repair the two types of cabling to the modules.

The repair to the CRPI (Control Rod Positie-Indication) modules involves an epoxy overmold to a distance great enough to cover the end of the existing cable and jacket (typically 3"-4").

The LVSI (Low Voltage Shielded Instrumentation) module repair adds two pieces of glj I

, shrink sleeve to each 3/c cable and an overmold of approximately 1"-l.5" of epoxy.

Both repairs are detailed on the attached drawing (attachment 1).

Due to the amount of additional epoxy added to the CRPI modules, a new sei:mic evaluation was performed for the reworked modules.

The evaluation concluded that the reworked module is not affected by the seismic forces as the strass developed is very small.

Since the CRPI module pigtail is the heaviest of all the module assemblies, all such reworked module assemblies would be qualified by similarity.

One Neutron Monitoring module will be shipped back to the manuf acturer for an overmold to correct a void.

The reworked penetrations are expected to be returned to the site by July 1, 1981 (except for the one Neutron Monitoring module).

Also, site QA has established an inspection hold-point prior to shipment by the manufacturer.

In as much as the review of the deficiency has been completed and rework initiated on each of the penetration types involved, this is considered a final report under 10 CFR 50.S5(e).

In the event there are questions relative to this report, please direct them to this office.

This supplemental report was scheduled to be submitted on May 15, 1981 but a delay to May 18, 1981 was granted by Mr. F. Reimann.
