Condition 1 - Licensee shall file with the Customs Officer or the Postmaster two copies,in addi-tion to those otherwise required of the Shipper's Export Declaration covering each excert and mark one of such copies for transmittel to 5% U.S. Nuclear Regu-lotory Commissicn, Washington, D.C. 20555. The following dedoration should oc-company or be placed on the Shipper's Export Dectoratiers for such exports:

"This shipment is being made pursuant to sce.fic oac number (specific

Condition 2 - Exports authorited in any country or destination, except Country Groups O. 5, W, X, Y, and Z vn Port 370, Supplement No.1, of the Comprehens ve Export Schedule of the U.S. Department of Commerce.

Condition 5 - Special nuclecr materici outhorized for export under th's 1 cense shall not be transported outside the United frutes in possenger carrying circraft in shipm.=nts exceeding (1) 20 grams or 20 cur'es whichever is less, of plutonium or uror :m 233, or (2) 350 grams of urcnium 235.

Cond%n 7 - The licensee sboll :cmplete and submit on NRC Form 741 for each shipment of source meter'oi exported under this license.

Condition 8 - The Ecensee shall advise the NRC in the event there is eny change in +he designa-tion of the company who will pcckage the nuc! ear metenal to be exported under th's license, or any change in the location of the packcging opemtion, at least three weeks prior to the scheduled dcte of export.

Condition 9-Shipments of plutonium by air, other than plutonium contained in a medical device designed for individual human application may only be made in packages the design of which the NRC has specifically approved for transport of plutonium by air.
