Follow-up on Headquarters Request 1.
Introduction On September 19, 1980, Washington Public Power Supply System (WPPSS) reported that Power Piping Co. Beam Attachment, HS-142, used in rigid sway brace assemblies were supplied to WPPSS Nuclear Project No. 2, which did not the meet the requirements of ASME Section III, Subsection NF, Mandatory Appendix XIII subparagraph XII-1732.1 in that, fillet. welds were observed to be undersized and of poor quality with lack of fusion at the toe of the weld.
Using ASME Code Allowables for analysis, it was also determined that a design deficiency exists for cegtain brackets when loads are applied at en angle greater than 15 off the perpendicular.

Inspection Objectives The objectives of this area of the-inspection were to review the nature and scope of the reported discrepancies and determine if the problem had gener'; implications.
3.
Method of iccomolishment The precrJ~ng objectives were accomplished by:
a.
Review of Liberty Equipment and Supply Co. Purchase Order No. N-86145TR dated 2/4/80.
b.
Review of Power Piping Company Drawing No. HS-142, Revision 9.
c.
Review of Pittsburgn Testing Laboratory Test Report No. 810694, dated 10/29/80, " Load Test of Fabricated Beam Attachments."
d.
Review of In-Process Control Sheet for the one Size 80 HS-142 beam attachment identified as being furnished to WPPSS-WNP 2.
e.
Review of inspection personnel visual examination qualifications.

Findings a.
Deviation From Commitments None.
b.
Unresolved Items Compliance of HS-142 Beam Attachment welds with ASME Section III, Subsection NF, Mandatory Appendix XIII, Subparagraph VIII-1742.1 could not be established during the inspection, as a result of the available information on discrepant products not being suf-ficiently specific with respect to identity and source of affected components.
c.
Comments (1) After being notified of the WPPSS Nuclear Project No. 2 concerns with respect to angular applied loads, Power Piping Company contracted with Pittsburgh Testing Laboratory for the performance of critical angle load tests on HS-142 beam attachments, sizes 2', 25, 40, 50, 60 and 80.
The results obtained for the size 60 and 80 beam attachments were below the values necessary for the assigned load ratings, with the other size attachments demonstrating satisfactory proof load valtes.
As a result of these tests, Power Piping has reduced the load rating for the two affected sizes and is assisting their only other nuclear customer for the size 60 and 80 sizes (Detroit Edison Company, Enrico Fermi Unit 2) in an evalua-tion of adequacy of attachment application.

It was noted that Purchase Order No. N-86145 TR from Liberty Equipment and Supply Co., for the supply of a size 80 sway strut to WPPSS Nuclear Project No. 2, did not provide any requirements or instructions with respect to component cap-ability for withstanding loads 15 degrees off the perpen-dicular.

Exit Meeting A post inspection exit meeting was held on October 30, 1980, with management representatives denoted in paragraph A. above.
The inspector explained the nature and scope of the inspection.
Manage-ment acknowledge the statements of the inspector and affirmed their cooperation to pursue and correct the problem as necessary.
