O APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-566 & 50-567 Yellow Creek License Nos. CPPR-172 &

CPPR-173 As a result of the inspection conducted on February 17-20, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.

A.

10 CFR 50, Appendix B Criterion IX, as implemented by TVA Topical Report TVA-TR-75-1A, Section 17.1A.9 requires measures be established to assure special processes, including welding and nondestructive testing are con-trolled. AWS D1.1 and ASME B and PV Code Section IX have been identified as the applicable code for welder qualification.

Visual inspe: tion and welding procedures compliance are controlled by TVA General Construction Specification G-29M.

Contrary to the above, on February 18, 1981, adequate measures were not established to control special processes including welding and nondestruc-tive testing in that the following were noted:

1.

A welder -was considered. qualified based on unacceptable bend test results.

2.

A welder failed to follow. bead width procedure requirements and a welding inspector accepted the unacceptable bead width on final visual inspection.

3.

This is a Severity Level V Violation (Supplement II.E).

B.

10 CFR 50, Appendix B, Criterion V, as implemented by TVA Topical Report TVA-TR-75-1A, Section 17.1A.5 requires that activities affecting quality be accomplished in accordance with documented procedures. TVA QC Instruc-tion C-207, Revision 4, " Reinforcing Steel Inspection", paragraph 3E1 and 3E3 require the minimum clear distance between parallel reinforcing bars and/or contact lap splices in a layer be greater than one inch.

Contrary to the above, activities affecting quality were.mt accomplished in accordance with documented procedures in that on February 17, 1981, two examples of improper reinforcing lap spacing were noted. In each case, four bars were spliced together without the one inch minimum splice separation.

This violation is applicable to Unit 1 only.

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
