Appendix A NOTICE OF VIOLATION Purdue University Docket No. 50-182 As a result of the inspection conducted on February 17-19, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
Technical Specification 6.2.3 states, "The Committee on Reactor Operations or a subcommittee thereof shall meet at least once per calendar quarter.
Contrary to the above no committee or subcommittee meetings were held between May 5, 1979 and October 17, 1979, a five month period.
2.
Technical Spr/cification 6.2.6 states, " Audits of facility activities shall be ps! formed..."
6.2.6.a. states, "The conformance of facility operations to the Technical Specification and applicable license conditions, at least once per 12 months."
6.2.6.b similarly requires an audit of training at least once per 12 months.
Contrary to the above, no audit was perfonned between October 1978 and May 1980, a 19 month period.
These are Severity Level V violations (Supplement IE).

Pursuant to the provisions of 10 CFR 2.201, Purdue University is hereby required to submit to this office within twenty-five days of the dite of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged item of noncompliance; (2) the reasons for the item of noncompliance if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further items of noncompliance; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, aa amended, this response shall be submitted under oath or affirmation.
