SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 - NRC-IE BULLETIN 79 FINAL INSPECTION REPORTS TVA's initial response to IE Bulletin 79-02 (Concrete Expansion Anchor Bolts) for Sequoyah Nuclear Plant was submitted July 5, 1979, and a revised response was submitted on December 6, 1979 During the NRC-0IE review of TVA's response, Inspector L. Modenos visited the Sequoyah site and identified two additional questions. A response to these questions was submitted on January 22, 1981. An additional six questions were identified by H. Asher (NRC-NRR) and the TVA response to these que.stions was submitted on February 1, 1980. The four responses mentioned above constitute the complete response to IE Bulletin 79-02 except for the anchor inspection final reports which are enclosed. l l l ENCLOSURE i Sequoyah Nuclear Plant Concrete Expansion Anchor Bolts Final Inspection Report The Sequoyah unit 179-02 anchor inspection program was initiated Septem-ber 29, 1979, to determine if TVA's quality control program met the requirements of NRC-OIE bulletin 79-02. A random sample of 30 safety-related pipe support base plates was selected for each size expansion anchor. The inspection program was performed and the data recorded in the same manner as was done for the Browns Ferry Nuclear Plant 79-02 program. A copy of the inspection criteria is included in attachment 1. The anchor inspection resulted in the evaluation of 139 anchors with the following deficiencies with regard to inspection criteria: 1. Three anchors (2.16 percent) had an excessive variance in plus depth. 2. Fifteen anchors (10.79 percent) had bolts with thread engagement less than 1 nominal bolt diameter required by the General Construction Specification G-32. A review of the inspection data with respect to structural integrity revealed only 3 potentially deficient anchors. The 15 bolts with thread engagements less than one nominal bolt diameter did not reduce the bolt strength below the capacity of the anchors. 'Only 1 of the anchors that exceeded the structural acceptance criteria would not develop the required anchor capacity. The failure rate for the sample inspection program was.72 percent. This is less than a 1 percent failure rate and compares very closely to the G-32 failure rate of.69 percent with 56 failures in 8174 pull tests. Conclusions The failure rate reflected by the 79-02 inspection data and the G-32 pull tests are comparable. The G-32 inspection program is confirmed, and therefore is representative of the total anchor installation at Sequoyah unit 1. The NRC has previously reviewed the gencric response, the 79-02 inspection results, and the G-32 results and had determined that TVA had complied with the requirements of bulletin 79-02 for unit 1 (NRC Safety Evaluation Report for Sequoyah Nuclear Plant Units 1 and 2, Supplement 2, Section 3.9 2). l i The Sequoyah unit 2 79-02 anchor inspection program was initiated September 8,1980, for the purpose of determining if TVA's anchorse . installations were represented by the G-32 quality control program. A random sample of 30 safety-related pipe support base plates was selected for ea6h size expansion anchor. The inspection criteria is identified in attachment 2. The anchor sampling inspection resulted in the evaluation of 217 anchors with the following deficiencies with regard to inspection criteria:' 1. Twenty anchors (9.22 percent) had an excessive plug depth variance. 2. Twenty-two anchors (10 95 percent) had bolts with short thread engagement. 3 Eighty-nine anchors had shells recessed deeper than 1/8-inch below the surface of the concrete. A review of the inspection data with respect to structural integrity reveals the following: 1. Two of the 20 anchors failed to pass the G-32 pull test. This constitutes less than 1 percent of the anchors inspected. 2. TWo of 22 with short engagement had less thread engagement than required to fully develop the anchors as determined by tests at Singleton Materials Engineering Laboratory. 3 With regard to deep shells, a TVA test program was performed to evaluate the effect of shear on the anchor with deep shells. The data indicated that recessed shells as much as 1/2-inch did not affect the capacity of the agchor. Results The.92 percent of anchora that were determined to be structually unacceptble compares favorably to the G-32 pull test data failure rate of.69 percent at Sequoyah Nuclear Plant with 56 failures out of 8174 pull tests. The inspection with respect to plug depth verifies the representativeness of the pull test program and no additional informa-tion is likely to be obtained by continued inspection for plug depth. The anchor installation with respect to both G-32 pull test data and 79-02 inspection data verified the anchor installations at Sequoyah unit 2 were structurally acceptable. There were only 2 bolts with thread engagements less than the one nominal bolt diameter required in G-32; however, the bolts had enough thread engagement to fully develop the anchor. 010330 38 1
i Conclusions .The results of the sample inspection are representative of the total anchor installation at Sequoyah unit 2. Since the 79-02 inspection is comparable with the TVA G-32 anchor installation and verification program, the G-32 program is represent-ative of the Sequoyah unit 2 anchor installation. - .2 Phase I of the inspection program shall be a random sample of a minimum of 120 base plates inisafety related systems. The plates are to be on ceilings, walls and floors, and are to be readily accessible. The sample population shall consist of a minimum of one anchor per plate with the following distribution:
(A) 30 each - Base plates with 1/2 inch diameter anchor bolt assemblies. j (B) 33 each - Ba,se plates with 5/8 inch diameter anchor bolt assemblies. I j (C) 30 each - Base plates with 3/4 inch diameter anchor bolt assemblies. . h.N. (D) 30 each - Base plates with 7/8 inch diameter anchor bolt assemblies. If more than four anchors are found to be outside of the anchor i acceptance criteria, Phase II of the inspection program is to be ini tia ted. 1.3 Phase II of the inspection program shall be a random sample of an additional 120 base plates in safety related systems.
