Gentlemen:
AP&L submitted our general plans for additional evaluations involving our failed fuel to determine the cause or causes of the failure in our February 26, 1981 submittal.
Subsequent telephone conversations with your Mr. Guy Vissing and Mr. Ralph Meyer on March 3 & 4, 1981 expressed the need for more detail in our submittal.
Based on that request, we are submitting this letter to document agreements made in the above-mentioned telephone conversations on future evaluations and reporting requirements regarding ANO-1 failed fuel.
As stated in our February 26, 1981 submittal AP&L plans to continue our evaluation to determine the cause of the failed fuel that occurred during Cycle 4 operation.
This evaluation will include further operating history reviews, fabrication record examinations and visual examinations above what has already been done. Operating history reviews will be re-evaluated in order to ensure nothing has been overlooked though now it seems probable that no situations occurred outside the parameters already defined as acceptable to preclude failed fuel. AP&L will independently review the fabrication records through our Middle South System QA inspectors in addition to the review which B&W has already performed.

Mr. Robert W. Reid, Chief from the core to ensure that the previous visual examinations did not overlook anything. The final detailed scope of work to be done on the above items will be submitted to the NRC on March 20, 1981.
Per previous agreements, AP&L will submit a status report concerning the above work on or before August 26, 1981.
If the above scope of work does not uncover the cause of the failed fuel that occurred during Cycle 4 operation, AP&L will continue to evaluate alternative methods to find the cause or causes as appropriate.
This further evaluation status will be described in our August 26, 1981 submittal and will include any alternatives that may prove worthwhile to the resolution of the concern. Such alternatives to be considered will include hot cell examinations, fuel disassembly and destructive examinations as appropriate.
For AN0-1 Cycle 5 operation, AP&L will notify the NRC Resident Inspector by the 10th day of each' month, in writing, as to the status of the equilibrium RCS activity levels from the previous months' operation. This note will discuss significant variations in equilibrium RCS activity levels and our estimation of how this might relate to the amount of failed fuel present.
