NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on December 1-5, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by docume.~.wd instructions, procedures, or drawings, of a type apprcpriate to the circumstances and shall be accomplistz1 in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitati;a or qualitative acceptance criteria for determining that important octivities have been satisfactorily accomplished."
Deviations from these requirements are as follows:
Stone & Webster's Engineering Assurance Procedure EAP 15.2, Revisfon 1,.2 (Instructions For Initiating, Dispositioning, and Approving N&D's) states in part "all blocks shall be filled in.
Enter 'N/A' when appropriate."
Contrary to the above, block 23 entitled "Related Activities Code" was not filled out by the responsible engineer for N&D reports No. 0406, No. 0445, No. 0497 and No. 0506.
Block 23 is the block where the responsible engineer indicates if a " Report of a Problem" is required, evaluation of a potentially significant deficiency (10 CFR 50.55(e)) is required, a licensing document is affected, or nothing is affected (N/A).
In a followup with the responsible engineer for N&D No. 0169 which was marked N/A and in which the nonconforn nce described therein was issued as a 10 CFR 50.55(e) report, the following was observed:
a.
Responsible engineer did not have or did not refer to the latest revision of EAP 15.2.
b.
Responsible engineer was not aware of his responsibility to fill out block 23 and indicated his belief that it pertained to test requirements.
The Millstone N&D file was examined before the completion of the inspection and ten (10) additional noncompliances were 'dentifiec and corrected.
- However, corrective action must be affected on any other projects affected by EAP 15.2 and preventive action has not yet been addressed.
